Monthly Subscription: Rs. 1,000/-

Introduction

Welcome to the PBS Office Gym! Our gym facility is designed to promote health and wellness among PBS employees. By registering for gym membership, you acknowledge and agree to abide by the following rules and regulations.

1. MEMBERSHIP & SUBSCRIPTION

• Monthly subscription fee is Rs. 1,000/- payable by the 5th of each month
• Subscription is non-refundable and non-transferable
• Members must carry their gym membership card during facility use
• Membership will be suspended if subscription payment is delayed beyond 10 days
• Three months advance notice required for membership cancellation
• Only PBS employees and authorized personnel are eligible for membership

2. OPERATING HOURS

• Monday to Friday: 11:00 AM - 8:00 PM
• Closed on Saturdays and Sundays and Public Holidays
• Members must vacate the premises 15 minutes before closing time

3. HEALTH & SAFETY

• Members must complete a health declaration form before first use
• Consult a physician before starting any exercise program if you have pre-existing health conditions
• Inform gym staff immediately in case of injury or medical emergency
• Use of equipment is at member's own risk; PBS is not liable for personal injuries
• Members with contagious illnesses must not use the facility

4. GYM ETIQUETTE & CONDUCT

• Wear appropriate athletic attire and closed-toe sports shoes at all times
• Bring your own towel and water bottle
• Wipe down equipment after each use with provided sanitizing wipes
• Return all weights and equipment to designated areas after use
• Maintain noise levels - avoid loud conversations and dropping weights
• Mobile phones must be on silent mode; phone calls should be taken outside
• No photography or videography without prior permission from management
• Sharing of equipment during peak hours - limit machine use to 20 minutes when others are waiting
• Respect personal space and privacy of other members

5. PROHIBITED ACTIVITIES

• Use of performance-enhancing drugs or banned substances
• Verbal or physical harassment of any kind
• Bringing outside food or beverages (except personal water bottles)
• Use of personal trainers not authorized by PBS gym management
• Inappropriate or offensive behavior towards staff or members

6. EQUIPMENT USAGE

• Only use equipment you are trained to operate safely
• Request assistance from gym staff if unsure about equipment usage
• Report any damaged or malfunctioning equipment immediately
• Do not modify or tamper with gym equipment
• Maximum weight limits must be observed on all equipment
• Spotters must be used when lifting heavy weights

7. CLEANLINESS & HYGIENE

• Dispose of waste in designated bins only
• Personal hygiene must be maintained at all times
• Report any cleanliness issues to management immediately

8. DISCIPLINARY ACTIONS

• First violation: Verbal warning
• Second violation: Written warning
• Third violation: Membership suspension for 1 month without refund
• Serious violations: Immediate membership termination without refund

. AMENDMENTS

• PBS reserves the right to modify these rules with 30 days notice to members
• Updated rules will be posted on the website and gym notice board
• Continued use of facility after amendments constitutes acceptance

By completing this registration, I acknowledge that:

 □ I agree to comply with all PBS Office Gym policies and procedures

 □ I understand that violation of these rules may result in membership termination

 □ I accept that PBS is not liable for any personal injury or loss of belongings

 □ I am physically fit to use gym facilities or have consulted a healthcare provider
